Submit supporting documentation to prove your pet’s rabies vaccination and microchip reading status. It is important to provide good quality pictures/scans of the supporting documents.

Clinical Record

We cannot accept clinical records sent by pet owners. To ensure authenticity and compliance, your veterinary practice must email your pet’s clinical records directly to us

This documentation is required to validate your pet's eligibility for Animal health certification. Incomplete or owner-submitted records will not be accepted.

  • Scanned Microchip
  • The Vaccine Manufacturer
  • The Vaccine Brand Name
  • The Vaccine Batch Number

Previously Issued AHC

If your pet has previously been issued an Animal Health Certificate (AHC) by an Official Veterinarian and the rabies vaccination recorded in that certificate is still valid, we can use the existing AHC to confirm your pet’s identity and rabies vaccination status. This may help speed up the process for your new AHC.

Pet Passport
 

Only properly completed pet passports can be accepted. The passport must include a valid (in-date) rabies vaccination, and the vaccination entry must be correctly certified.

Rabies Vaccination Entry – Required Details:

To certify the rabies vaccination properly, the following must be recorded in the passport:

  •  Veterinarian's Name
  •  Veterinarian's Signature
  •  Practice Address
  •  Practice Telephone Number

Rabies Vaccination Card Requirements 

To accept a Rabies Vaccination entry on a vaccination card, all of the following information must be clearly provided and correctly certified:

Practice Stamp – The official stamp of the veterinary practice.

Vet’s Name – Clearly printed or written.

Vet’s Signature – Must be original and legible.

Microchip Number – The animal's unique microchip number.

Confirmation of Scan – A clear statement that the microchip was scanned at the time of vaccination.
